Leveraging Appreciative Inquiry and data-driven insights, Common Thread facilitated clear communication channels, strategic alignment, and actionable plans to enhance service delivery across Marion, Polk, and Yamhill counties. Josephine County Prevention & Treatment Services (JCPT)

Empowering Youth Through School Outreach

Common Thread collaborated with Josephine County Prevention & Treatment Services to evaluate and enhance the School Outreach Officer (SOO) program, a pioneering initiative aimed at supporting youth in local schools. By conducting comprehensive surveys, focus groups, and site visits, Common Thread assessed program effectiveness, identified gaps, and developed a roadmap for improvement. The project emphasized building stronger partnerships between schools, families, and community organizations while addressing critical issues such as truancy, substance abuse, and student wellness. Through tailored strategies and actionable recommendations, Common Thread helped position the SOO program as a model for prevention and intervention services in Oregon.

The project included developing strategies for workforce development, community outreach, and data-driven decision-making to support sustainable growth. Common Thread collaborated with CCWIC to develop the 2023–2024 Program Roadmap, focusing on transforming the WIC program into a model of excellence for Clackamas County. Through an in-depth evaluation process using Appreciative Inquiry and a Balanced Scorecard framework, Common Thread identified strengths, addressed limitations, and crafted strategic recommendations to enhance equity, accessibility, and family-centered care. The roadmap includes actionable plans for improving team dynamics, streamlining operations, fostering innovation, and expanding community engagement, ensuring sustainable growth and meaningful impact for families in the region.

Common Thread provided strategic project management and consulting to the Traditional Health Worker Alliance, an initiative established by the Willamette Health Council. By coordinating stakeholder meetings, creating a comprehensive charter, and guiding task prioritization, Common Thread ensured alignment and momentum throughout the project. The team also supported grant applications, developed supervision tools to meet OHA and Medicaid requirements, and conducted a workforce survey to inform growth strategies. This collaborative effort strengthened the Alliance’s capacity to enhance community health outcomes through targeted, sustainable initiatives.

Common Thread’s role included facilitating strategic planning, coordinating partners, and developing tools to enhance program integration and impact. This initiative ensures THWs are well-prepared to improve community health outcomes while building resilient, inclusive care networks.
